Requisition 15:
Professional, ethical, and respectable representation of BFMC/CCPN/HPN & Heritage California ACO to Patient and Provider Community
Obtain and document all pertinent patient information in q. ACO.
Efficiently capture ACO Quality Measures on each visit/during each phone call.
Task all incoming message to the appropriate IDT member.
Maintain patient safety and privacy.
Reinforce and support patient education programs.
Follow-up with beneficiaries, over the phone, as delegated by the Case Managers
Document all beneficiary interactions in q. Call
Request/obtain medical records from providers & medical facilities, as directed by the Case Managers and upload into q. Doc accordingly.
Complete Patient Health Questionnaires (Depression Screen, Home Safety Eval., etc.) with beneficiaries and fax positive findings to providers as needed.
Maintain spreadsheet to track medical record requests
Send welcome letters, patient education materials, unable to contact letters, etc. to beneficiaries and providers when directed to do so by the Case Managers.
Maintain open/effective communication with Case Managers and Administrative staff.
Must be able to multi-task, prioritize, and stay organized.
Assist with training and orientation of new personnel.
Identify problems, provide solutions and evaluate results as needed.
Initiate/participate in Code White, Code Blue, Code Red situations as they arise within guidelines of scope of practice.
Be familiar with contents on the G Drive, Intranet, including Policy and Procedures and MSDS.
Certified Medical Assistant ACO
Requirements: FT Graduate from accredited MA program with current State Certified Certificate. Current CPR.
Required 45 WPM certified typing certificate.
Two (2) years experience in related field.Estimated Salary: $20 to $28 per hour based on qualifications.

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
