The Clinical facility mainly works with the low income, uninsured community members with special emphasis on homeless people, migrant and seasonal farm workers and public housing residents.

Summary
The CSS is responsible for the daily operation of the back office staff while also serving the providers to the fullest utilization of licensure. The CSS works in collaboration with the Care Team to ensure efficient and quality patient care.

Performs clinical duties, maintains and operates medical and laboratory equipment, assisting in the care of patients, triage calls and walk in's as well as monitor and track employee performance as it relates to clinical practice and safety.

It is the primary purpose of CHC to provide the highest quality, cost effective healthcare to the patient population that it serves.

Essential Duties and Responsibilities
Participates in patient population management as it relates to clinical services with taking into account cultural diversity and local resources.

Participates in the team member's care coordination by providing follow up phone calls, pre-visit checklists, Assists with follow up appointments within the recommended time frames. Triage phone calls and walk in services when necessary.

Under the role in self-management support by developing relationships with high risk clients and anticipate what needs they may encounter. Develop the ability to refer to other team members (LCSW, MD, Providers) when clinical judgment is in question.

Develop communication skills with all members of the team both internal and external, by first being a patient advocate and use the available resources to facilitate care. Coordinate with the Care Team (PCP, LCSW, DON or Medical Director) to ensure appropriate services are arranged for the patient in order for them to meet their self-management goals.

Understand the quality improvement process which may include meeting, data collection and chart auditing. Use Evidence Based Practice guidelines to assist with disease management.

Verified patient information and adheres to all HIPPA guidelines. Assists with reports in i2i and Nextgen.

Responsible for orientation, training and supervision of back office staff. Responsible for Case Management and closed loop tracking of labs, referrals, tests and follow up. Delegates responsibilities to support staff based on the staff licensure or qualifications.

Qualifications
- Current California RN License
- BSN Preferred
- Two (2) years experience
- Knowledge of current regulatory and compliance guidelines as they relate to the facility's Mission, Vision and Values
Estimated Salary: $20 to $28 per hour based on qualifications.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
